I only wanted forehead Botox: do I need glabellar treatment too? Why both are assessed in Korean consultations

When you want forehead Botox, there may be a medical reason to assess the glabella too: the relationship between facial muscles and eyebrow movement. However, a “2-area package” on a menu does not establish that everyone needs both areas treated. During examination, have the physician check actual forehead and glabellar movement, baseline eyebrow and eyelid positions and expressions you want to preserve, and explain the exact product instructions and reason for the recommendation.
Distinguish anatomical planning from simple bundling when an extra area is recommended
Even if only horizontal forehead lines are visible, eyebrow-raising and frowning movements should be assessed together. But the number of wrinkles in a photograph or areas on a quotation cannot be treated as medical necessity. During consultation, first check what changes at rest, when raising the eyebrows and when frowning, and ask about the separate reason for glabellar treatment and the limitations of not having it. Definitions and how the treatments work
Botulinum toxin temporarily reduces signaling in selected muscles. The forehead and glabella differ in movement and function and are not a single area treated with identical locations and units. The US BOTOX Cosmetic instructions recommend treating the glabella with the forehead to reduce the possibility of brow droop. This is that product’s US guidance and must not be extended to mean authorization or doses for all Korean products or a mandatory purchase condition for everyone.
Potential benefits and limitations
The aim is to soften suitable expression lines while considering desired movement. A 2023 study analyzing 3-dimensional facial expressions in 30 healthy adults showed varied movement of adjacent areas and between sides when moving the forehead. It was not a clinical trial comparing satisfaction and safety of treating two areas versus one, so it cannot establish that two areas are better for everyone. Botox also cannot resolve all skin sagging, major volume loss or deep fixed wrinkles.

Disclose existing eyebrow asymmetry, eyelid droop, heavy-feeling eyes or dry eyes, previous toxin products, areas and dates, and expressions you wanted to preserve. Procedure, recovery and scheduling
During examination, the physician assesses the frontal view and facial expressions and explains the product, treatment extent, expected changes, functional risks and total cost. After consent, disinfection, injections, observation and provision of records take place. Small injection marks, bruising and swelling may occur, and the final response is not judged immediately. Check photography, sightseeing and departure plans and assessment methods in advance; do not independently add injections based on remaining movement. Obtain activity instructions such as exercise and cleansing tailored to the actual treatment.
Side effects, contraindications and symptoms requiring medical attention
Brow or eyelid droop, asymmetry, unwanted muscle weakness and eye discomfort are possible, as are bruising, pain, headache and allergy. Treating both areas does not remove these risks. Report heavy-feeling eyes or uncomfortable expressions to the clinician after treatment; difficulty swallowing, speaking or breathing, or generalized weakness requires immediate medical care.
