Is pico toning more effective than melasma creams? Criteria for choosing a laser before visiting Korea
Full translation of the Korean article. Translation checked for completeness, numerical consistency and medical meaning; separate medical review of this translation is pending. Translation updated: 2026-10-28.

Pico toning is not always more effective than every melasma cream. Diagnosis, photoprotection and a medication plan are important for melasma, while lasers may be an option or adjunct for some patients. Actual comparative trials also compare specific devices and methods with a drug at a particular concentration; they must not be generalized into a conclusion that pico replaces creams.
Three pathways for deciding whether to add a laser in Korea
Ask in sequence whether the condition is truly melasma, how topical treatment and photoprotection have actually been used, and whether assessment and recovery can continue if a laser is added now. Uncertain diagnosis, dermatitis or irritation require medical assessment and stabilization first. If the existing plan is helping, a trip does not itself create a need to add a laser. If melasma remains despite adequate care and conditions are suitable, consider adding treatment after discussing expected benefits, PIH risks and follow-up. A 2023 randomized trial of 60 people compared two pico methods with 2% hydroquinone; all improved, but group outcomes differed. This does not represent every pico treatment, other concentrations, combination formulations or individual responses. Do not copy the trial’s medication washout period or laser settings as your own preparation instructions.
| Current situation | What to confirm with the clinician | What not to overlook in the decision |
|---|---|---|
| Uncertain whether it is melasma | Distinguish solar lentigines, post-inflammatory marks and other causes | Do not interpret dark-spot studies as melasma effects |
| Accompanying irritation or redness | Assess current products, dermatitis and recent procedures | Do not try to solve it with stronger treatment |
| Improving with existing care | Maintenance plan, goals and assessment timing | Unnecessary additions during travel may be omitted |
| Remains despite appropriate care | Additional benefits and risks of the actual device and mode | Not a single solution requiring all creams to be stopped |
| Intense sun exposure or return home is imminent | Feasibility of monitoring, photoprotection and local medical care | Postponement is also an option if the schedule does not fit |
| Pigmentation worsens after treatment | Differentiate PIH, recurrence, hypopigmentation and other causes | Do not automatically proceed with remaining package sessions |
Definitions and how the treatments work
Pico is a category of short-pulse lasers, and responses vary with wavelength, optics and method. Topical melasma treatments regulate pathways such as pigment production according to their ingredients and require prescribing, duration planning and irritation management. Hydroquinone, combination formulations and other ingredients differ, and lasers and medications do not have identical roles.
Potential benefits and limitations
Pigmentation measures may improve in selected patients, but recurrence and uneven responses are possible, and permanent removal is not guaranteed. The 2023 trial randomized 60 people and analyzed data from 59, with limitations in skin types, treatment conditions and follow-up. Favorable results in a particular pico group are not used to represent all melasma patients or claim lasers are always faster. The AAD describes photoprotection and topical treatments as the foundation, with procedures considered as needed.
Who can seek a consultation and how suitability is assessed
Adults with diagnosed melasma who can continue daily care and follow-up assessment may seek consultation. In addition to skin tone, disclose previous PIH or hypopigmentation, recent tanning, irritation or lasers, current whitening agents, retinoids or photosensitizing medications, pregnancy, breastfeeding and allergies. Rapidly changing, bleeding or asymmetric lesions require differential diagnosis before cosmetic procedures.
Procedure, recovery and scheduling
Record current products, duration of use, reactions and melasma distribution, and obtain the rationale for continuing, changing or adding laser treatment. Redness, stinging and swelling may remain for several days or longer after laser treatment; crusts and prolonged color changes are possible depending on the method. Do not mix creams yourself or use high concentrations for long periods; follow clinician instructions on stopping and restarting. Also confirm cleansing, makeup and sun-protection methods, sightseeing routes and assessment timing after returning home.

Side effects, contraindications and symptoms requiring medical attention
Laser risks include burns, PIH, hypopigmentation, worsening melasma, infection and scarring; topical agents also have ingredient-specific risks such as irritation and contact dermatitis. Do not prepare for a laser by stripping the skin with strong creams. Have blisters, severe pain, pus, fever, new white spots or prolonged darkening assessed. Do not independently add oral whitening medications or tranexamic acid. Actual adverse-effect frequencies vary by device, method and individual condition.
The comparison is not “expensive lasers versus cosmetics”
| Category | Role and goals | What to check |
|---|---|---|
| Diagnosis and photoprotection | A foundation for reducing triggers and worsening pigmentation | Do not infer necessary care from season or nationality alone |
| Clinician-planned topical treatment | Ingredient-specific pigment regulation | Distinguish ordinary cosmetics from prescription treatment |
| Selective addition of a laser | Device- and mode-specific approaches to remaining concerns | Assess evidence for single and combined treatment, recovery and recurrence |
| Postpone first | Address irritation, diagnostic uncertainty and difficulty maintaining care | A safe plan rather than a missed opportunity to visit |
Questions and answers for a medical consultation
These consultation questions were compiled from the literature and do not represent a clinician interview or an individual prescription. Clinical review of the new article has not yet been completed.
Q. Can I stop melasma medication if I have pico treatment?
A. No. The clinician must determine whether to continue, stop or restart based on existing ingredients, skin response and treatment method.
Q. Is it better to have a laser immediately since I am visiting Korea?
A. A visit itself is not an indication. First check diagnosis, current response, sun-exposure plans and follow-up feasibility.
Q. If it was better in a clinical trial, will it be the same for me?
A. The results concern comparisons of particular products, methods and participants. They do not guarantee the same effects with other creams, devices, skin conditions or a single visit.
Consultation with the SEOLREIM clinic director and signature programs
SEOLREIM Clinic states its principles of physician-led consultations and procedures, genuine products, accurate quantities and attentive care. Consultation starts by distinguishing skin condition and goals, then checking the products and devices actually available, approved indications, dose or energy plans, recovery and risks. During consultation, have the SEOLREIM physician first distinguish melasma, dark spots and skin irritation and confirm the actual device and prescription. Skin-quality programs such as Shining Rejuran are not described as mandatory components of melasma treatment or equivalent substitutes for lasers and medications. Decide on adding a program after checking its separate goals, ingredients and risks.
Booking, languages, location and guidance for international visitors
When viewing a pigmentation treatment menu at a dermatology clinic in Korea, ask separately about melasma, freckles and sun spots. Bring photographs of recently used products, treatment records and plans for sun exposure after Korea; confirm your desired consultation language and interpretation support before booking. SEOLREIM Clinic is on the 11th floor of the Gangnam8258 Building, approximately 1 minute from exit 3 of Gangnam Station. Book through the website’s KakaoTalk consultation, Naver booking or telephone channels. International visitors comparing dermatology clinics in Gangnam, Seoul, near Gangnam Station or elsewhere in Korea should prepare information on medications, allergies, recent procedures and departure dates. When booking, state your preferred language, such as Japanese, Chinese, Arabic, Mongolian or Indonesian, and whether interpretation is needed, and confirm the actual scope and cost of support. Continuous consultation availability in each language and same-day procedures cannot be guaranteed. During consultation, also agree on how to contact the clinic about adverse reactions after returning home and the follow-up schedule.
