Are pico toning and pico fractional the same procedure? How to check modes before booking for melasma or dark spots
Full translation of the Korean article. Translation checked for completeness, numerical consistency and medical meaning; separate medical review of this translation is pending. Translation updated: 2026-10-28.

Pico toning and pico fractional do not mean the same procedure even when the same pico device is used. Actual optics, energy distribution, settings and goals may differ. If you want improvement of melasma or dark spots, check before booking whether an option also includes pore or texture goals, whether that addition is necessary and its recovery burden.
Read a pico package in three lines: goals, mode and recovery
First prioritize pigment lightening or changes in texture and depressions. Then request separate explanations of toning, targeted dark-spot exposure and options using fractional optics. Names such as focus, lens and honeycomb can mean different things across clinics and devices and are not standardized names for identical settings. Adding a fractional option does not automatically improve melasma treatment. If a pore option is added on site to a toning booking, check the extra purpose, recovery, cost and alternative of declining it before deciding.
| Question | What to check | Common misunderstandings |
|---|---|---|
| What is being treated? | Diagnosis of melasma, freckles, lentigines or PIH | Assuming all brown pigmentation is the same |
| Which mode is used? | Toning, targeted exposure or fractional options | Assuming the same device name means the same procedure |
| Why add an option? | Separate texture goals beyond pigmentation and supporting evidence | Assuming a lens makes treatment better for melasma |
| How does recovery differ? | Redness, swelling, flaking, crusts and observation | Assuming every pico procedure requires no recovery |
| What is included in the quotation? | Areas, mode, anesthesia, aftercare and additional costs | Assuming toning and every option are automatically included |
Definitions and how the treatments work
Pico is a pulse-duration category; actual wavelength, optics, spot, output and placement determine treatment. Fractional delivery distributes energy into microscopic sections but must not be simplified as having the same action as other ablative lasers. In a 2021 split-face melasma trial, a particular lens option did not provide additional pigment improvement over conventional optics. Recent studies also compare specific conditions and do not establish rankings across all modes or skin types.
Potential benefits and limitations
Treatment may aim for changes in pigment or texture appearance in selected candidates, but evidence and assessment differ for each goal. Melasma tends to recur, making photoprotection and prescription management important. Adding fractional treatment does not guarantee fewer necessary sessions or a melasma cure, and paper findings are not applied to a tourist package merely sharing a device name.
Who can seek a consultation and how suitability is assessed
Adults with a pigmentation diagnosis who can maintain photoprotection and follow-up may seek consultation. Disclose recent tanning or sunburn, dermatitis, wounds, herpes, previous PIH or hypopigmentation, photosensitizing medications, recent peels or lasers, pregnancy and breastfeeding. Skin tone is not inferred from nationality alone; mixed concerns are assessed as separate treatment goals.
Procedure, recovery and scheduling
After examination and photographs, determine the actual mode, areas, eye protection, anesthesia, assessment and stopping criteria. Mild toning, fractional and targeted treatment may differ in the extent of redness, swelling, flaking and crusts and may require several days or longer to recover. Follow instructions for makeup, photography, sightseeing and exercise based on actual settings and responses. A short procedure name does not imply short recovery or follow-up care.

Side effects, contraindications and symptoms requiring medical attention
Besides redness, dryness and swelling, burns, blisters, PIH, hypopigmentation, worsening melasma, rare scars and infection are possible. Adding options may accumulate irritation, so review previous responses. Report blisters, oozing, severe pain or progressing color changes to the clinician; do not pick peeling skin or add strong peels.
Distinguish three pico menu options by goals and settings
| Category | Role and goals | What to check |
|---|---|---|
| Toning | Exposure planning matched to pigment concerns | Diagnosis, actual settings and response assessment |
| Targeted dark-spot exposure | Treatment focused on individual lesions | Crusting and recovery may differ from toning |
| Pico fractional option | Fractional energy distribution and separate goals | Check optics, indications, PIH and the need for additions |
Questions and answers for a medical consultation
These consultation questions were compiled from the literature and do not represent a clinician interview or an individual prescription. Clinical review of the new article has not yet been completed.
Q. Does the same pico device mean the same pain and recovery?
A. No. Modes, settings, areas, skin responses and anesthesia differ.
Q. Is a fractional option better for melasma?
A. That conclusion does not apply to everyone. Check evidence for that mode and individual suitability.
Q. Can I omit the option if I do not need pore treatment?
A. Confirm available components and prices with the clinic, but do not assume an additional procedure without a separate goal is mandatory.
