Ultherapy versus Thermage: if choosing just one in Seoul, what should you compare when recommendations differ?

When choosing Ultherapy or Thermage, first identify the area and change you want rather than the more famous device. If two clinics make different recommendations, ask for explanations of the diagnosed problem, expected changes, areas to avoid, recovery and total cost on the same basis. This does not mean one device is better for every face or that both procedures are mandatory together.
When device recommendations differ, compare their rationale
If one clinic recommends Ultherapy and another Thermage, neither is automatically wrong. Even for the same jawline, they may assess mild laxity, fat distribution, existing hollowing or substantial excess skin differently. First choose the single area you most want to change, then ask the physician what problem was identified and what changes the device can or cannot achieve. A 2019 split-face trial of 20 people found improved laxity measures with both ultrasound and monopolar RF, with no statistically significant difference between methods. A small trial does not establish equivalence of all latest models or the best individual choice. Instead of claims such as “deeper is superior” or “RF is safe for everyone,” check area-specific reasons and conditions for deferring treatment.
| Question | Answers to record | Answers needing further clarification |
|---|---|---|
| Main problem | Is it skin laxity, contour, fine lines or volume? | A device name without an explanation of the diagnosis |
| Goals and limitations | Which area may look different, and by how much? | Surgery-like changes or resolution of every concern |
| Plan | Actual model, areas, zones to avoid and consideration of previous procedures | Only citing a high shot count |
| Recovery and scheduling | Pain management, expected reactions, follow-up visits and departure | Guarantees of no pain, no side effects or immediately complete results |
| Quote | Included items and additional costs for each procedure | Converting shots from different devices into the same amount |
| Deferring the choice | When expectations do not fit or the diagnosis is unclear | Pressure to have both today |
Definitions and how the treatments work
Ultherapy is a microfocused-ultrasound platform with visualization, while Thermage is a monopolar-radiofrequency platform. Energy delivery differs; device names or a one-sentence description of tissue layers cannot establish the positions of skin, fat and nerves. Check the actual model’s authorized uses and treatment plan.
Potential benefits and limitations
Gradual changes in the appearance of mild skin laxity and wrinkles may be expected in selected adults, but removal of substantial excess skin, volume augmentation or a replacement for surgery cannot be guaranteed. Distinguish early swelling from the final course, and do not interpret no difference in a comparative trial as identical results for everyone.
Who can seek a consultation and how suitability is assessed
Adults seeking nonsurgical changes are assessed for skin thickness, laxity, fat volume, existing hollowing and goals together. Disclose implanted electronic devices, metal, altered sensation, infection or wounds, fillers, threads, surgery, past energy-based procedures, pregnancy and breastfeeding. In particular, Thermage’s contraindication for implanted electronic devices cannot be resolved by a simple price comparison.
Procedure, recovery and scheduling
After examination by the physician and recording areas, obtain explanations of the actual device, tip or cartridge, pain management, eye protection and treatment extent. Redness, swelling, bruising, tenderness and sensory changes may persist for several days or longer. Do not judge success or failure solely from immediate photographs on the final day of travel; establish a follow-up assessment plan.

Side effects, contraindications and symptoms requiring medical attention
Burns, blisters, scarring, surface irregularities, persistent pain, sensory or nerve abnormalities and volume changes have been reported. Postmarketing reports in official Ultherapy instructions cannot precisely establish incidence or causality; they do not mean everyone develops cheek hollowing or that Thermage causes no volume changes. Seek immediate medical care for weakness, visual changes, progressing asymmetry or severe pain.
Differences to have explained when choosing one treatment
| Category | Role and goals | What to check |
|---|---|---|
| Ultherapy | Planning using focused ultrasound and visualization | Target areas, actual depth, tissue interpretation and risks |
| Thermage | Monopolar RF thermal responses and skin-appearance goals | Contact, cooling, tips and contraindication for implanted electronic devices |
| Combination or postponement | A separate goal actually exists, or the timing is unsuitable | Assess each treatment’s individual benefits, risks and total cost separately |
Questions and answers for a medical consultation
These consultation questions were compiled from the literature and do not represent a clinician interview or an individual prescription. Clinical review of the new article has not yet been completed.
Q. What if one clinic recommends Ultherapy and another Thermage?
A. Compare each clinic’s diagnosed problem, expected changes, areas to avoid and limitations on the same basis, and request examination by the physician.
Q. If a small comparative trial found no difference, can I choose either?
A. No. Study participants, devices, settings and assessment periods are not the same as individual consultations or all latest models.
Q. Do I need both for more certain results?
A. Combining treatments does not itself guarantee results. Ask about the separate goal the additional procedure addresses and its risks and cost.
