A 1927nm thulium fractional laser delivers water-absorbed light into microscopic skin zones to target superficial pigment, photodamage, and texture. It does not permanently remove melasma; redness, flaking, post-inflammatory hyperpigmentation (PIH), and recurrence must be considered. A device name such as Lavieen or the 1927nm wavelength alone does not guarantee identical settings, depths, or effects.

02

Definitions and how the treatments work

Fractional delivery leaves skin between small treatment zones rather than treating all skin identically at once. Microzone responses to a 1927nm device depend on energy, density, passes, and skin condition; describing every device as necessarily nonablative or reaching the same depth is inaccurate. Principles and skin responses differ from melanin-targeted pico toning; cosmetic absorption and collagen effects also require device-specific data.

03

Potential benefits and limitations

In a 2020 randomized split-face pilot study for melasma, both the side receiving topical TXA after laser and the control-solution side improved, with no significant difference between sides at 3 months. It is therefore difficult to conclude that laser-assisted TXA delivery always adds benefit. A 2023 photodamage study was prospective but nonrandomized and does not guarantee effects in every melasma case or dark skin tone. Photoprotection, causal-factor management, and maintenance are important.

04

Suitable candidates and preliminary assessment

Adults concerned about surface pigment, rough texture, or photodamage first need lesion diagnosis and skin-response assessment. Disclose recent tanning, repeated PIH or hypopigmentation, dermatitis, infection, herpes, healing problems, photosensitizing medicines, and recent other lasers or peels. A mole changing size, color, or shape or bleeding requires diagnosis before cosmetic laser treatment. Elective procedures and accompanying ingredients during pregnancy or breastfeeding need separate consultation.

05

Procedure, recovery and preparation

After checking pigment type, skin tone, tanning, and history, plan the actual device, settings, need for a test area, anesthesia, and eye protection. Obtain instructions on soothing care, moisturizer, sunscreen, and flaking care, and review recovery photographs. Do not rub off redness-associated rough skin or tiny crusts. Surface recovery may take several days to about 1 week or longer, while pigment changes need longer follow-up; adjust strong sun, swimming, events, and reassessment during travel.

06

Adverse effects, contraindications and precautions

Besides pain, erythema, swelling, dryness, and flaking, burns, blisters, infection, herpes reactivation, PIH, hypopigmentation, and rare scars are possible. A test spot or low energy does not guarantee safety. Seek care if pain or heat increases or blisters, pus, or spreading erythema appear. Applying arbitrary ampoules under occlusion after laser changes irritation and absorption risks, so clinicians must check accompanying ingredients.

Comparing 1927nm fractional treatment, pico toning, IPL, and topical medication

Comparison of procedure differences

Pico toningShort-pulse approach to selected pigment lesionsConsider melasma recurrence and hypopigmentation risks from repeated treatment

Questions and answers for a medical consultation

The answers below are literature-based consultation questions, not a clinician interview or individual diagnosis; actual medical review remains incomplete.

Q. Will Lavieen remove melasma in one session?

A. Melasma may recur; wavelength and device name alone cannot determine session numbers or results.

Q. Is applying tranexamic acid after treatment better?

A. A specific study found no significant difference from the control side. Check the combined formulation, absorption, and safety separately.

Q. How much extra time should I allow for a thulium laser trip to Korea?

A. Depending on settings and skin response, surface recovery may take several days to 1 week or longer. Plan sun exposure, reassessment, and returning home after examination.

SEOLREIM’s principle of consultations and procedures performed personally by the director

SEOLREIM Clinic describes consultations and procedures performed personally by the clinic director, with principles of genuine products, appropriate doses, and attentive care. Consultations begin by distinguishing skin condition and goals, then checking the products and devices actually available, approved uses, dosage or energy plans, recovery, and risks. The signature melasma and spot program first distinguishes lesions, inflammation, and skin tone and plans maintenance. Confirm current ownership and provision of 1927nm devices and Lavieen when booking.
