Particulate PCL fillers such as Ellansé use carrier gel and polycaprolactone microparticles to supplement volume and induce gradual tissue responses. Unlike HA filler, PCL particles cannot be dissolved with hyaluronidase; first check management of overcorrection, nodules, and vascular problems and suitable areas as well as duration. Even under the same PCL ingredient name, soluble formulations and particulate fillers cannot be treated as equivalent.

01

Definition, benefits and precautions at a glance

Key consultation points
CategoryWhat to check
DefinitionBiostimulatory filler using carrier gel and PCL particles
GoalVolume and wrinkle supplementation in selected areas and gradual tissue responses
PersistenceVaries by actual product formulation, area, and individual
RemovalHA dissolvers do not remove PCL particles
Main risksNodules, inflammation, overcorrection, infection, vascular occlusion, and vision injury
02

Definitions and how the treatments work

The carrier gel contributes to initial volume in particulate PCL fillers, while tissue responses around particles may contribute to later changes. Formulations determining degradation rate and persistence differ; ingredient names alone do not establish how many years effects last. Do not directly apply particulate Ellansé research to other PCL forms such as Gouri, or advise identical injection techniques and suitable areas.

03

Potential benefits and limitations

A randomized comparative study assessing nasolabial folds reported correction with a specific PCL product. However, findings for one product in one area cannot be generalized to under-eyes, lips, neck wrinkles, or all skin tones. They do not mean an identical collagen response in every individual or that aging stops. A conservative plan is important: allow early swelling to settle and assess progress before deciding whether more is needed, rather than immediately adding product.

04

Suitable candidates and preliminary assessment

Adults being assessed for selected volume loss or wrinkles who understand removal limitations and can attend follow-up may seek consultation. Disclose active infection or skin inflammation, ingredient allergies, previous filler nodules or unidentified permanent fillers, bleeding tendencies, anticoagulants, immune disorders, pregnancy, and breastfeeding. Use outside approved areas needs separate explanation; do not automatically apply it to thin or highly mobile areas such as under-eyes or lips.

05

Procedure, recovery and preparation

Assess previous fillers, surgical history, and vascular risk; obtain explanations of the actual product, formulation, areas, total amount, expected persistence, and emergency response. After disinfection and any necessary anesthesia, the clinician injects with a suitable technique and checks early skin color and pain. Swelling and bruising may last days to weeks, requiring plans to avoid treatment just before important events. Follow product-appropriate care and reassessment advice rather than self-massage or requesting extra correction immediately after injection.

06

Adverse effects, contraindications and precautions

Besides swelling, bruising, and tenderness, asymmetry, overcorrection, palpable nodules, delayed inflammation, infection, and granulomas are possible; rare vascular occlusion, skin necrosis, and vision injury can occur. Severe pain, pallor, reticular discoloration, or visual abnormalities require emergency assessment. Claiming that hyaluronidase also removes PCL is inaccurate; nodule treatment depends on infection, inflammation, material, and location, and removal can be difficult or require surgical assessment.

Differences between particulate PCL, HA, and PLLA fillers

Comparison of procedure differences

Particulate PCLInitial volume and gradual tissue responsePCL particles cannot be dissolved with HA dissolvers
HA fillersFormulation-specific volume, contour, and hydration goalsEven dissolvability does not guarantee immediate complete removal
PLLA SculptraGradual collagen responseReconstitution, techniques, and nodule management differ

Questions and answers for a medical consultation

The answers below are literature-based consultation questions, not a clinician interview or individual diagnosis; actual medical review remains incomplete.

Q. Is Ellansé always better because it lasts longer?

A. Persistence may also mean harder correction and removal. Judge by goals, suitable areas, and follow-up arrangements.

Q. Can PCL fillers be dissolved with hyaluronidase?

A. PCL particles are not removed by HA dissolvers. Nodules or overcorrection are treated after verifying the actual formulation and cause.

Q. Can I leave immediately after Ellansé treatment in Korea?

A. First arrange early checks, complication contacts, and reassessment. Adjust the stay to individual recovery.
