Low facial fat is not automatically a contraindication to HIFU such as Ultherapy. However, uniformly applying the same depth and shots to a face primarily lacking volume rather than showing laxity may accentuate hollows. Before treatment, check fat volume, skin thickness, bone contours, nerve paths, and actual cartridge depths, and obtain an explanation of why existing hollows should be avoided or energy and placement adjusted.

01

Definition, benefits and precautions at a glance

Key consultation points
CategoryWhat to check
Distinguish firstSkin laxity, fat descent, naturally low volume, and bone structure
Planning factorsActual device and cartridge depth; placement and overlap rather than shot count alone
LimitationsTightening does not fill hollow cheeks
Possible abnormalitiesPain, swelling, altered sensation, rare fat atrophy or nerve abnormalities
Immediate assessmentOne-sided weakness, severe pain, blisters, or progressive indentation
02

Definitions and how the treatments work

Microfocused ultrasound concentrates ultrasound energy at selected depths, creating small thermal coagulation points to induce tissue remodeling. A cartridge’s stated depth does not guarantee the same anatomical layer in everyone; skin, fat, and nerve positions vary in cheeks, temples, and jawline. Even devices with ultrasound imaging have limits in visualization and operator interpretation.

03

Potential benefits and limitations

Systematic reviews report limited improvement in facial/neck laxity and mostly temporary pain, redness, or swelling. Studies differ in devices, assessments, and follow-up, and exact frequencies of rare subcutaneous fat atrophy or nerve injury are difficult to predict. Low facial fat alone does not establish better or worse effects; lifting cannot fill existing hollows.

04

Suitable candidates and preliminary assessment

People with mild laxity but little fat may discuss conservative planning. Report previous HIFU, RF, filler, thread lifts, facial surgery, one-sided facial paralysis/neurologic disease, dental/skin infection, reduced sensation, pregnancy/breastfeeding, and implants. Recent unexplained hollowing, pain, or swelling calls for diagnosis before additional energy treatment.

05

Procedure, recovery and preparation

Assess volume and laxity before and during expression and divide areas such as temples, anterior/lateral cheeks, and jawline. Record actual devices/cartridges, ultrasound visualization, areas to avoid, shot overlap, energy, and goals. Pain, swelling, bruising, or temporary sensory changes may follow; decide on strong massage or further energy treatment only after checking recovery and clinical instructions.

06

Adverse effects, contraindications and precautions

Besides pain, redness, swelling, bruising, and tenderness, burns, blisters, pigment changes, reduced sensation, temporary strength changes, and rare nerve injury/fat atrophy are reported. Seek prompt care for suddenly worsening asymmetry, difficulty speaking/smiling, persistent tingling or pain, blisters, or skin-color changes. Aggressively reducing normal volume with a “slimming lift” goal may be difficult to reverse.

07

Comparing treatment goals when facial fat is low

Comparison of procedure differences
MethodMain goalKey differences and precautions
HIFUThermal coagulation and tightening at selected depthsCheck fat amount, nerves, cartridges, and overlap
Noninvasive RFImprove laxity through electrical-resistance heatCheck device-specific depths, cooling, and implant contraindications
Fillers/volume treatmentSelective volume replacement in hollowsCheck vascular complications, overcorrection, and existing fillers
Observation without treatmentObserve natural bone structure, volume, and age-related changesMay be reasonable for minor laxity

Questions and answers for a medical consultation

The answers below are literature-based consultation questions, not a clinician interview or individual diagnosis; actual medical review remains incomplete.

Q. Should I avoid Ultherapy if I have no facial fat?

A. It is not a blanket contraindication, but assess whether volume loss exceeds laxity and which areas/depths to avoid or adjust.

Q. Does avoiding deep cartridges prevent cheek hollowing?

A. Stated depth alone cannot guarantee safety. Assess regional skin/fat thickness, shot placement, overlap, and energy together.

Q. Should filler be injected immediately if cheeks hollow after treatment?

A. First assess swelling changes, fat atrophy, and muscle/nerve problems; decide on correction once the cause is stable.

SEOLREIM’s principle of consultations and procedures performed personally by the director

SEOLREIM Clinic explains its principles of consultations and procedures performed directly by the clinic director, genuine products, exact quantities, and attentive care. Consultations first distinguish skin condition and goals, then check products and devices actually available, approved indications, dosage or energy plans, recovery, and risks. For signature lifting such as Ultherapy, the director distinguishes skin laxity from fat/bone structure and plans genuine tips, cartridges, and sites. Check device availability and individual shot/energy suitability during examination.

Booking, language, and location information for overseas visitors

Visitors seeking Ultherapy in Korea or Gangnam dermatology should share recent filler/HIFU records, baseline-volume photos, and departure date, and obtain adverse-reaction contact and follow-up plans. SEOLREIM Clinic is on floor 11 of the Gangnam8258 Building, approximately 1 minute from exit 3 of Gangnam Station. Book through the website’s KakaoTalk consultation, Naver Booking, or telephone channels. Overseas visitors comparing dermatology clinics in Gangnam, Seoul, near Gangnam Station, or elsewhere in Korea should prepare their medications, allergies, recent procedures, and departure schedule. When booking, specify the preferred language, such as Japanese, Chinese, Arabic, Mongolian, or Indonesian, and whether interpretation is needed, and confirm the actual scope and cost of support. Continuous consultations in each language or same-day procedures cannot be guaranteed. Also agree during consultation on how to report adverse reactions after returning home and the follow-up schedule.
