RF microneedling for pores and acne scars: how does it differ from fractional lasers?

RF microneedling inserts fine needles into the skin and delivers radiofrequency energy at or around their tips to target dermal remodeling. Results and recovery depend on needle depth, energy, skin tone and scar depth. Treating active acne must be distinguished from treating scars.

Definition and mechanism
Fine needles deliver radiofrequency energy into the skin to create thermal stimulation in the dermis and encourage collagen remodeling during recovery. Devices differ in needle insulation, depth and energy delivery. Therefore, treatments sharing the name ‘RF microneedling’ do not necessarily have the same protocol or recovery.
Potential benefits and limitations
Treatment may aim to improve the appearance of enlarged pores, texture, some atrophic acne scars and fine lines. Deep narrow scars, ice-pick scars and active inflammation may require another treatment or a combination. It does not permanently close pores or remove scars in a single session.

Infection, persistent redness, burns, hyperpigmentation, hypopigmentation or worsening scars may occur rarely. Disclose active acne or herpes, medications that increase bleeding risk, keloid tendency, immune suppression and previous adverse laser reactions. At-home microneedling devices should not be considered equivalent to medical treatment.
How it differs from other treatments
Fractional lasers create microscopic thermal injuries using laser energy; RF microneedling mainly delivers energy through needles. Pore injections deliver ingredients into the skin, while PDT and medication address active acne inflammation and sebum through other mechanisms. Planning should distinguish scar types from current inflammation.
