PDO and PCL thread lifts: effects, duration and differences from energy-based lifting

A thread lift places absorbable medical threads beneath the skin to physically support tissue and use the surrounding tissue response to improve contours. Results and risks depend on thread material and barbs, insertion depth and the causes of facial sagging.

Definition and mechanism
Absorbable threads, such as PDO or PCL, are inserted into specific tissue layers to support sagging tissue and adjust its direction. Some use barbs or cones to engage and pull tissue; a fibrous tissue response may develop as the threads are absorbed. Complication risks can rise when thread choice or insertion depth is unsuitable for the face's vascular and nerve anatomy.
Potential benefits and limitations
Mechanical support from the threads may initially make facial contours look more defined, with tissue responses contributing over time. However, this does not remove substantial excess skin or correct deep sagging in the way a surgical facelift can. Duration varies with thread type, sagging, lifestyle and assessment timing, so a uniform guarantee is difficult.
Who may be a candidate
People mainly concerned about mild sagging around the jaw or cheeks and reduced contour definition may seek consultation about an option with less recovery burden than incisional surgery. Very thin skin, substantial sagging, larger facial fat volume or asymmetry from other causes may require a different approach first.
Procedure and recovery
The direction of sagging, skin thickness, fat volume, asymmetry and previous filler or thread treatments are assessed. The insertion route is planned with local anesthesia; tenderness, swelling, bruising and pulling sensations may persist for days to weeks afterward. Early restrictions may include opening the mouth widely, strong massage, saunas and strenuous exercise.

Side effects, precautions and contraindications
Bruising, swelling, pain, pulling, temporary asymmetry and dimpling can occur. Less commonly, threads may become exposed, or infection, inflammatory nodules, altered sensation or nerve irritation may develop. Active infection, significant bleeding risk, uncontrolled chronic illness, pregnancy and breastfeeding require medical assessment before treatment.
How it differs from other treatments
Ultherapy uses focused ultrasound and Thermage uses radiofrequency to trigger thermal responses in skin and tissue. Thread lifts use inserted threads for physical support and tissue responses. Selection should depend on the layer and direction of sagging, skin thickness and desired recovery rather than simply asking which option is ‘stronger’.
Medical Q&A
Q. Does the effect disappear as soon as the threads dissolve? A. Absorption, tissue responses and changes in sagging follow different timelines, so this cannot be assumed. Q. Can energy-based lifting be combined with threads? A. The medical team determines the sequence based on insertion depth, skin response and recovery. Q. Is it normal to feel a thread? A. An early foreign-body sensation may occur, but persistent or worsening symptoms need assessment.

SEOLREIM's approach and signature programs
SEOLREIM CLINIC assesses the direction of sagging, skin thickness, fat distribution and facial asymmetry rather than focusing solely on pulling in one direction. When threads are appropriate, their role is distinguished from Ultherapy, Thermage FLX, Oligio and Benjamin Lifting to avoid excessive combinations. Direct physician consultation and follow-up are central to the approach.
